Wellness Navigation

Looking at the Whole Person

Wellness Navigation looks beyond one immediate need.

For example, someone may initially ask for help finding transportation to an appointment. Through conversation, we may also learn that they are having difficulty accessing groceries, are spending a lot of time alone, or would like to become more active.

The Navigator can help the person look at these needs together and decide what is most important to them.

This is sometimes called social prescribing, connecting people with non-medical supports and community resources that can have a positive impact on their health and well-being.

The individual remains at the centre of the process. We work with people on the goals they identify for themselves.

For Health Care Providers and Community Partners

Wellness Navigation can be a referral option when a senior, older adult or Elder has identified social or practical needs that are affecting their health and well-being and would like support beyond medical care. With their consent, a referral can be made to Elder Connect to help explore available resources, services and community supports that may address those needs. You do not need to know which community program or service is the right fit before making a referral. That is part of the Navigator’s role.

We can help explore available options, connect the individual with appropriate community resources and, where needed, support them in making that connection.

Wellness Navigation can also help identify situations where a needed service does not currently exist or is difficult to access locally. These gaps help inform Lillooet Learns’ broader work with health care providers, Indigenous communities, local organizations, government and other partners to strengthen services and referral pathways in the Lillooet area.

Supporting People to Age Well in Their Community

The goal of Wellness Navigation is to help seniors, older adults and Elders remain healthy, connected, independent and involved in their community.

Health is influenced by much more than medical care. Transportation, food, housing, relationships, culture, safety, activity, purpose and belonging can all affect a person’s ability to live well.

By helping people navigate these supports, and helping organizations work better together, Wellness Navigation creates a stronger connection between health care and community care.

Make a Referral or Ask for Support

You can contact Wellness Navigation for yourself, a friend, someone you support, or to discuss whether a referral may be appropriate.

You don’t need to know exactly what service you need before contacting us. We can start with a conversation and figure out the next steps together.
